There are several forms that are required by the State of New Jersey, including the ALL MEDIA PARENTAL PERMISSION FORM, the ANNUAL POLICY NOTIFICATION, and the ACCEPTABLE USE POLICY. 

If you have not completed the Media Permission form, your child’s name, and photo, cannot be included in any district publication, including year books and school newsletters.

If you have not completed the Acceptable Use Policy form, your child will be denied access to our schools network, and computer resources.  This will affect your child’s ability to complete assignments in class, as well as participate in online assessments.
